By checking this box I (i) affirm that I have reached the age of majority in my state of residence at the time of this RSVP entry, and, (ii) as the primary named registrant and parent/guardian of the secondary named minor registrant, authorize such minor registrant to participate in this event and authorize NPCA to collect personally identifiable information from such minor registrant and to use and disclose that information in accordance with NPCA’s Privacy Policy available at www.npca.org/privacy.
